Richard's Story

Richard was diagnosed with Mitral Valve Regurgitation, with one requirement being to halt exercise until he received results from his cardiologist. Despite his conditions, remaining active is his favorite hobby, so this was frustrating news to receive. 

When talking about his diagnosis and upcoming specialist appointments, he became stressed and frantic. I asked how he was processing everything—and if he needed to pause and take a deep breath. He took the time to collect himself and thanked me for asking about his emotional well-being.

He couldn’t reach the cardiology office and waited weeks for results. I intervened and got the results within a couple of days, after which he was able to resume exercising. 

I wanted him to know that I understood where he was coming from, and that I was there for him to make this experience as seamless as possible. He knows now that he doesn’t have to navigate his chronic conditions alone, and I’m just happy to be there for him.

Carl was initially hard to engage with, unwilling to open up, and often didn’t take his medications as prescribed. I built rapport with him through positive reinforcement during our monthly calls to establish trust. We decided to tackle one issue at a time to build his confidence.

We started by focusing on his Type 2 diabetes, discussing how a healthy diet and staying consistent with medications could help lower his glucose levels. Within six months, he nearly cut his A1C in half. This was huge progress and his attitude completely shifted. After seeing such drastic, measurable improvements, he feels empowered.

Her Care Navigator says…

Althea’s HBA1C was at 12.5 and she was experiencing chronic sciatic pain. She was frustrated, not improving with physical therapy, and felt like nothing helped. It is hard to stay motivated when you’re uncomfortable 24/7.

We first focused on her blood pressure, and I shared the benefits of a low-carb diet—plus creative ways to keep her meals exciting. We also discussed the importance of staying consistent with physical therapy and additional exercise. 

She was prescribed medication that required prior authorization, which can be discouraging and intimidating, but we worked together on it, and I arranged for her to get samples from her doctor in the meantime. Slowly but surely, her blood sugar levels stabilized. 

Seven months after we started working together, her A1C decreased from 12.5 to 6.3, she lost 12 lbs, and she was fully cleared for spinal surgery. Today, she’s better managing her diabetes and is also on the path to resolving her chronic pain issues.
