At SelectPatient Management, we're transforming the way individuals with chronic conditions manage their health in between visits to the doctor. Our Care Navigators offer customized support for patients with multiple chronic conditions such as diabetes, hypertension, and obesity. Each month, they'll check in to see how you re doing, but can also help you better manage your medications, schedule appointments, set goals, stay motivated, and so much more.
Care Navigators do not make medical decisions for patients, but do communicate all clinical needs directly to physicians.

Only 20% of managing your chronic condition happens through clinical care, but we’re here to support you through the remaining 80%. Our Care Navigators are dedicated to helping you get and stay healthy.
Regular Check-Ins & Availability
Your Care Navigator will reach out at least once each month, but are available to you as often as you need.
We Work With Your Doctors
Your Care Navigator will share information back to your doctor(s) via your electronic health record, to ensure everyone is up to date on your progress; including you.
